If nothing is done to reverse the epidemic, IDF predicts that by 2030, 438 million people will live with diabetes at a cost projected to exceed US$490 billion.

Faced with these alarming numbers, World Diabetes Day aims to establish access to diabetes education as a right for all people with diabetes, to promote greater awareness of the risk factors and warning signs of diabetes, and encourage best-practice sharing in diabetes prevention.

This year sees the second of a five-year campaign (2009-2013) that addresses the growing need for diabetes education and prevention programmes throughout the world. The theme for World Diabetes Day 2010 is “Let’s take control of diabetes. Now.” In keeping with this theme, IDF has developed a special web application – the Blue Circle Test-to showcase the risk factors for type 2 diabetes and highlight the positive actions that individuals can take to help prevent the disease.

FINALLY, GLOBAL POLITICAL WILL FOR A PERMANENT SOLUTION

The world is finally waking up to the threat of diabetes and other non-communicable diseases (NCDs). On May 13 this year, the UN General Assembly voted unanimously for UN Resolution 64/265 to hold a UN High Level Summit on Non-Communicable Diseases in September 2011. A similar Summit on HIV/AIDS in 2001 proved a turning point for the fight against that disease.

“The UN Summit will bring heads of state, government representatives, NGOs and public health experts together to discuss the global threat and commit to the global response required for diabetes and other NCDs,” Ann Keeling, IDF Chief Executive Officer said. “World Diabetes Day provides an ideal platform to disseminate and promote IDF’s key messages and objectives for the UN Summit,” Ms Keeling added.

To mark World Diabetes Day this year, IDF is coordinating a programme of work – called the Diabetes Roadmap – that will produce and package arguments, evidence and solutions to ensure the UN Summit translates into real change for the millions of people with diabetes worldwide.

A Call to Action on Diabetes will be launched on November 14 in the diabetes epicentre of the world: China. China has 92.4 million adults with diabetes (1 in every 10 adults). A Call to Action is the central advocacy tool for the global diabetes community in the lead up to the UN Summit, bringing the global diabetes epidemic to the attention of world leaders, and guide and secure action, commitment, support and resources for diabetes. The document will be unveiled as part of World Diabetes Day festivities at the iconic Great Hall of the People in Beijing. New data on how much diabetes is costing the Chinese economy and society will also be announced.

Celebrations will extend from China to various countries in the world, with famous landmarks and monuments once again lighting up in blue for diabetes awareness. More than 500 iconic buildings in over 60 countries will be illuminated, including Table Mountain in South Africa, Christ the Redeemer in Brazil, the Brandenburger Tor in Germany, and Niagara Falls in Canada. Activities such as mass walks for diabetes and aerobics will be held in conjunction with these lightings, to demonstrate the importance of a healthy lifestyle to help take control of this epidemic.

Helping Patients with Multiple Sclerosis

New experimental procedures – Liberation Treatment and Plasma Exchange – bring new hope to patients suffering from Multiple Sclerosis

October 25, 2010 /India PRwire/ — Multiple Sclerosis (MS), also known as disseminated sclerosis or encephalomyelitis disseminate is a neurological disorder, in which a human body loses its ability to communicate the neural signals from the brain to the spinal cord.

A patient suffering from MS can experience varied symptoms, which vary from loss in sensitivity, muscle weakness, trembling, and difficulties in ambulation, chronic pain or even visual problems.

There are various types of MS. However, there are three types that have been observed most commonly, viz., Relapsing Remitting Multiple Sclerosis (RRMS) – most common form of MS in which the patient tends to experience series of attacks followed by complete or partial remission; Secondary Progressive Multiple Sclerosis (SPMS) – starts with RRMS, with progressive attacks and no real periods of remission or recovery from symptoms; and Primary Progressive Multiple Sclerosis (PPMS) – most commonly found in men, with no real periods of remission and the condition deteriorates with time.

MS has not yet been proven as a genetic disorder. However, a number of genetic variations have shown increasing propensity towards developing MS. Other risk factors include decreased Vitamin D production due to lack of sunlight exposure, mainly in areas far away from equator; severe stress; smoking; and other environmental abnormalities.

In scientific terms, MS is an inflammatory disease in which the body’s immune system eats away the protective sheath that covers the nerves. This interferes with the communication between the brain and the rest of the body. Ultimately, this may result in deterioration of the nerves themselves, a process that’s not reversible, in majority of cases.

Presently, there is no treatment protocol approved for MS. Experimental trials of procedures such as Liberation Treatment and Plasma Exchange have shown positive results in patients suffering from MS. There are medications (mostly anti-cancer) that are also prescribed in the preliminary stage of MS. However, most doctors believe that these medications are symptomatic drugs and in some cases, can also lead to adverse effects or be poorly tolerated.

Drugs prescribed to slow down MS progression

  1. Beta interferons: Mainly prescribed to a patient who suffers one attack per year of a relapsing form of MS or don’t recover well from neurological attacks. It is also recommended in cases where a patient develops new lesions or shows increasing tendency towards severe symptoms
  2. Glatiramer: Mainly prescribed in cases of RRMS
  3. Natalizumad: Prescribed as monthly treatments to subside the frequency of MS attacks
  4. Mitoxantrone: Prescribed to patients suffering from aggressive RRMS or SPMS

Liberation Treatment

According to a whitepaper – “Multiple Sclerosis International Federation (MSIF) Statement, Chronic Cerebrospinal Venous Insufficiency (CCSVI)”, MSIF – recent experiments have shown that a phenomenon called as Chronic Cerebrospinal Venous Insufficiency (CCSVI) has shown a reported abnormality in blood drainage from the brain and the spinal cord. CCSVI is believed to contribute to the nervous system deterioration leading to damage causing MS. The risks and benefits of procedures to treat CCSVI have not been established by properly controlled clinical trials. A recent study, by Dr. Paolo Zamboni from the University of Ferrara in Italy, on 65 people with different types of MS, reported abnormal venous flow in 100% cases. The researchers also noted that patterns of venous obstruction differed between people at different stages and courses of MS although there was no clear relationship between severity of MS and extent of venous obstruction. However, further studies are still underway in order to conclusively establish a relationship between CCSVI and MS.

Liberation Treatment – a surgical procedure for treating CCSVI – has also shown positive results in a small number of patients suffering from MS. In this procedure, a balloon dilation is used to open up obstructed veins or insertion of stents into veins to help keep them open (endovascular surgery). Liberation Treatment, just like other endovascular procedures, also carries risks, such as elastic recoil, rupture of the vein and blood clots, etc. However, that does not mean that these procedures wouldn’t be established as potentially safe and effective treatment.

Plasma Exchange

According to an article – “Plasma Exchange, HealthNow New York Inc.”, Plasma exchange (PE) is a procedure in which the plasma is isolated, then discarded and replaced with a substitution fluid such as albumin. Plasma exchange is a nonspecific therapy, since the entire plasma is discarded. PE has been used in a wide variety of conditions including the treatment of neurological diseases. Laboratory abnormalities suggest that MS is an immune-mediated disease. PE has been used primarily as a technique to either shorten the duration of an acute attack or to reduce the number of acute attacks in patients suffering from MS. In plasma exchange, doctors remove the blood and mechanically separate the blood cells from the plasma (fluid). The blood cells are then mixed with a replacement solution, typically albumin or a synthetic fluid with properties of plasma, and are returned with the blood cells in the body.

Treatment of MS in India

According to the study, Road traffic Accidents (RTAs) and Fall From Height (FFH) are the two most common causes of spinal injuries, accounting for 43.5% (n=320) and 38.04% (n=280) of the total cases respectively.

The study also compared the collected data with similar studies undertaken in developed countries. The analysis revealed that road traffic accidents are the most common cause of spinal injuries even in the west. However, ‘fall from height’ was not as prominent as in India. Sports injuries and violence were found to be more important causes of spinal cord injury in USA.

“The results of the study will help us develop better prevention and treatment methods in India. It is very crucial to keep an eye on injury patterns as these can provide a hint for devising solutions like specialized treatment, and setting up speciality centres according to the local needs,” says Dr. H.S Chhabra.

The study reveals that spinal injury in India results more often in paraplegia (paralysis of the two limbs below the torso) than tetraplegia (paralysis of all four limbs). The study shows that 65% of patients became paraplegic as opposed to 46.2% in the US. This may reflect a better situation for the Indian patient since it is easier to manage a paraplegic than a tetraplegic. However, more patients in India have “complete injuries” (which carry a prognosis that they cannot be overcome neurologically)-i.e. 69.25% as compared to 39.8% in USA.

The male female ratio (5.6:1) is much higher in the study as compared to that in other studies from across the globe (3.8:1). This is probably due to the fact that most Indian women are housewives and are not predisposed to injury. A significantly higher percentage of SCI were married at the time of injury i.e. 63.18% (n-719) as compared to statistics available from USA (30.6%). This probably reflects a lower age at which most Indians get married.

The study revealed that the age distribution of patients was more or less the same as that of developed countries except for the fact that the proportion of elderly patients (age more than 60 years) was significantly lower (5%) in developed countries (9.8% in USA).

The importance of epidemiological studies in planning prevention strategies is well established. It is also well known that epidemiology of a particular ailment varies from region to region since it is linked to social, environmental, cultural and biological issues. However, despite its importance, there are hardly any proper epidemiological studies from developing countries. Thus countries like India have information only from a few hospital based pilot studies which has varied in different studies depending on the region and the rural/urban population catered by the hospital. However such studies suggest that there may be significant epidemiological differences in India as compared to other developed countries.
